* If it is your first visit to this hospital, you cannot make an appointment in advance. Please come to the hospital during the registration hours.

*Even though you have been to this hospital before, if you wish to see a doctor at a medical department that you have not attended before, you are regarded as a first visit patient.

1. Collect a "Registration Form" and fill it out. Submit it to #1 counter with your Japanese national health insurance card if you have one.
2. Please wait until your name is called from counter #1

1. Please go to the department that was designated by the reception and wait until your name is called.
2. While you are waiting, a nurse will bring you a medical questionnaire. Please answer all questions and submit it to the outpatient department reception counter.

*Please note that patients with appointments have priority.

Please collect a plastic folder from a nurse after consultation finishes. 

1. Please submit your folder to counter #3-#6.
2. You will receive a payment ticket here. Please wait until your ticket number is displayed on the TV screen near Counter #6.
3. If your doctor has made your appointment for the next time, the appointment slip is also issued here.

• You can pay by cash or credit card.
• Rental fee for crutches should be paid by CASH ONLY.
• A medication ticket is issued here if your doctor has prescribed medicine.

 Please wait until your number or a number greater than your number is displayed on the TV screen near Counter #6, then go to the automatic payment machine behind the waiting area to make a payment. The machine has a language selection button at the bottom left. Press “English” to hear English instructions.

on the electric bulletin board next to the pharmacy (Counter #9).

Please submit your medication ticket here
if you have one and receive your medicine.
