1. Patient’s recovery and health maintenance
2. Improvement of medical and nursing care services

・Patients’ personal information is not used for purposes other than the above without prior written notification and consent of the patient.

• The hospital does not provide personal health care information to third parties without prior consent of the patient.
• If the purpose is for the cases stated in the Section 1, Articles 1 to 3, we provide the patient’s personal information to third parties in order to provide necessary medical services unless otherwise requested by the patient.

 

• The hospital implements appropriate security management measures in order to prevent the leak, loss, or damage of personal information. 

•  In providing medical/nursing care services, this hospital outsources part of the examination, reception/insurance claim, food services and other work, and these parties may handle patients’ personal information. In such cases, the hospital takes supervisory responsibility to ensure the proper protection and management of personal information.

• A patient has the right to disclose, correct, make additions or deletions, or stop utilization of their personal information.
• For detailed procedures, contact the Department of Medical Security Management. Costs related to the issuance of the medical record for disclosure of the patient’s personal information are charged (e.g. 20 yen per page).
