Inpatient Admission Procedures
On the day of admission, please come to the Inpatients Reception（⑧ on the floor map） at the appointed time.
You will need the following things for the admission procedure.
- Admission application form
- Application for rental hospital gown/pajamas/towel/diaper
- Admission questionnaire
- Hospital ID card
- Inpatient handbook
- Health insurance card (If you have one)
- Claimant certification for medical care (If you have one)
